When you use our contact or referral forms, we collect the details you type in — your name, contact information, and your message. Form submissions are delivered to the clinic’s email inbox via Web3Forms, a form-delivery service; they are not stored in a database on this website. We ask that you do not include detailed medical information in website forms — clinical details are best shared at your appointment or, for physicians, by fax.
Payments
Online payments are processed by Stripe on Stripe’s own secure pages. We never see or store your card details on this website.
Analytics
We use privacy-friendly, aggregate analytics to understand how the site is used (for example, which pages are visited). This data does not identify you personally.
Links to other services
Some pages link to partners such as Nymble Health and Stripe. Those services have their own privacy policies that apply once you leave this website.
Your medical records
